Sales Tax Exemption

If your organization is tax exempt we must have a valid Certificate of Exemption from your state department of revenue (or other related state goverment office) on file. Before we schedule your service call for your organ. If we don't have the certificate on-file when we schedule your service call you WILL be charged sales tax (where applicable). We are NOT able to go back later and modify the invoice for your service. This is due to various rules related to sales tax collection in various states.

BE SURE that you submit the form listed below for your location. We must have the form listed below to remove taxes from your invoice/s. We can not accept any other form than the one listed here for you state.

We must have an updated form for your organization for each new calendar year.

Minnesota

Please complete MN Department of Revenue Form ST3, Certificate of Exemption :

Be sure that you complete all the required fields, especially mark that your organization is tax exempt and upload the completed PDF form of your tax exempt form.

Enter this information:

  • Project Description: Organ Repair
  • Name of Purchaser: Name of your organization
  • Business Address: The physical location of the organization (where the organ is located)
  • Purchaser's Tax ID Number: (your tax ID number)
  • Name of seller: 61KeyService LLC
  • Seller's Address: Box 9207 St. Paul, Minnesota 55109
  • Type of Business: check box 20 (other) and enter "Church" on the line
  • Reason for Exemption: check box G (religious organization) and enter your exempt number
  • Complete the info at the bottom and sign (needs to be an authorized signer for the organization)
    NOTE: We do NOT recommend checking the first box ("Check if this ... is for a single purchase..."). If you do so and you need any follow-up service within the same year, or if your issue requires 2 visits, then you will have to submit a new form for EACH service call.

Wisconsin

Please complete WI Department of Revenue Form S-211, Wisconsin Sales and Use Tax Exemption Certificate:

Be sure that you complete all the required fields, especially mark that your organization is tax exempt and upload the completed PDF form of your tax exempt form.

  • Purchaser Information: Your organization's information
  • Seller Information:
    • Name: 61KeyService LLC
    • Address: Box 9207 / St. Paul / MN / 55109
  • In the Other section
    • Check the box 'Other purchases exempted by law'
    • Enter "Church" in the box on that line
  • Complete the name/title/date area and Sign in the Signature of Purchaser (needs to be signed by someone authorized to sign for the organization)

Iowa

We must have a copy of your Certificate of Exempt Status from the IRS and/or your notification of exemption from the Iowa department of revenue. This must be the completed and approved/final form, not your application for such. Only tax exempt organizations that are also religious institutions (using the organ for worship) or educational institutions are able to get tax exemption for services and parts we provide.

South Dakota

We must have a copy of your South Dakota​ Certificate of Exemption form approved by the State of South Dakota. We can not accept any application forms, only completed forms. Contact the South Dakota Department of Revenue and complete their application process. Once you are approved and have the Certificate of Exemption we will need you to provide a copy of that certificate.

NOTE: Churches are NOT exempt from sales tax in South Dakota

North Dakota

We must have a copy of your Certificate of Exempt Status ​from the North Dakota department of revenue. You must first file an Application for Sales Tax Exemption Certificate with the department and have that approved. Once you are approved and have the Certificate of Exempt Status we will need you to provide a copy of that Certificate. We can NOT accept a copy of the Application form.

NOTE: Churches are NOT exempt from sales tax in North Dakota
